Responsibilities include: Completing admissions, social service evaluations, providing referrals, and maintaining communication with residents and families. Assisting with financial and legal matters such as pensions, legal referrals, and preplanning arrangements. Providing discharge planning services, including coordinating placements and working with outside agencies. Arranging counseling services and psychiatric medication management as needed. Ensuring accurate and timely documentation in MDS care plans. Qualifications: Current LPN or RN licensure in the state Preferred: Current social work licensure in the state Bachelor’s degree in social work required; master's degree preferred One year of supervised social work experience in healthcare preferred About Ciena Healthcare: Michigan’s largest provider of skilled nursing and rehabilitation services, committed to compassionate, personalized care.
